for Remote Patient Monitoring

ViTel Health, Inc. is searching for a new member on its clinical health team. The ideal candidate is a personable, people-oriented, licensed Clinical Health Care Professional, who can provide coaching and patient oversight for remote patient monitoring and chronic care management. Our company is focused on monitoring and managing vital signs to minimize critical health events. A ViTel Health Care Coach will develop close relationships with their patients to: set up the devices, train the patient, monitor and manage notifications, and set up telemedicine encounters with the primary care physician. They will be focused on encouraging and motivating the patient to consistently take their vital sign readings, adhere to the plan of care, and refer to the provider as needed.
