Health Care Coach

ViTel Health, Inc is searching for a new member of its clinical health team who is a personable, people-oriented, licensed Clinical Health Care Professional to provide coaching and patient oversight for remote patient monitoring and chronic care management.

Our company is focused on monitoring and managing vital signs to minimize critical health events. A ViTel Health Care Coach will develop close relationships with their patients to: set up the devices, train the patient, monitor and manage notifications, and set up telemedicine encounters with the primary care physician. They will be focused on encouraging and motivating the patient to consistently take their vital sign readings, adhere to the plan of care, and refer to the provider as needed.

 Candidates must have:

• The ability to practice legally across state lines with a primary residence in a Nurse Licensure Compact State
• Current California State licensed as an LPN, Medical Assistant, Social Worker, Pharmacy Tech, Nursing assistant, Social work tech, etc.
• Demonstrated people and communication skills, with experience in customer service
• Demonstrated experience learning and using new software applications
• Demonstrated team oriented attitude
• 2-years work experience, preferably in Chronic Care Management
• Technology savvy
• Experience in remote work
• Understanding of Chronic Care Management and Remote Patient Monitoring

We prefer the following interests, training, experience, and background.

• Training in RPM and CCM requirements, components, and billing
• Understanding of SDOH
• Interest in leadership roles and desire for future career growth 
• Able to obtain multi-state licenses as necessary
• Background, education, or interest in nutrition, integrated medicine, and/or lifestyle medicine
